Note:   AT&T California and AT&T Nevada POTS Resale Residence:  2 hour appointment will be offered to residential customers if the initial 4 hour appointment and the subsequent 4 hour appointment are missed due to company reasons.  CLEC will receive a jeopardy and can accept the 2 hour appointment or send a SUPP with a new requested 2 hour appointment in Remarks.  The standard 2 hour window will be (only if the second 4 hour appointment is missed):  10 am – 12 pm, 2 pm – 4 pm, 4 pm – 6 pm.
