Manual Pre Ordering Forms
Effective with business on Monday, November 15, 2010 AT&T will begin accepting the Customer Service Inquiry (CSI), PreOrder and Access Service Request (ASR) forms via Email. As a result of this, a revised set of forms will be used replacing the existing Bar-coded CSI, PreOrder and ASR forms. There will be a six month introduction period for the CLECs to migrate from Fax to Email from November 15, 2010 through end of business June 17, 2011. After the seven month grace period has ended, the manual forms will no longer be available for use.
